Liability Forms and Processes

 

If you have a Galaxy Digital account, adult volunteers only complete a liability waiver once for all volunteer activities at all Tennessee State Parks within a calendar year. You will be prompted to sign one after responding to an opportunity for the first time that year. It will be kept on file electronically in our system. You will be qualified for future volunteer opportunities until December 31st when you will be asked to complete another one for the new calendar year. 

If you had previously responded to an opportunity and need to update your waiver, go to your profile and choose Qualifications on the menu.  Under Incomplete Qualifications click the link to sign the new one. 

If you do not have a Galaxy Digital account, adult volunteers must complete the paper version of our liability waiver each time they volunteer at a Tennessee State Park.  You can download the waiver form here.  You won't be able to respond online to any opportunity without an account.  Please notify the site supervisor by email that you plan to attend and bring the completed waiver with you. 

Minor children must have their parent or legal guardian complete a consent form each time they participate in a volunteer opportunity at a Tennessee State Park. You can download the consent form here. Please notify the site supervisor by email that you plan to attend and bring the completed consent form with you. Minor children with accounts won't be able to respond to online opportunities with a waiver qualification due to system limitations. Please remember that children 15 years old and younger must have a parent, legal guardian, or other responsible adult present while volunteering.

Tip: If you are an adult with a Galaxy Digital account and volunteering with others like your family, friends, or co-workers, consider creating a Team when responding to an opportunity.  This will allow you to complete your waiver and reserve spots for the rest of your party.  Reserved spots can be anonymous (great for minors or those not wanting accounts) or you can invite others to your Team by providing email addresses.
